direct your care, there needs to be an honest relationship between

program that will be used by you when treating your diabetes. The
plan will be based on physical exam, test results, and your
input.
You will be seeing your doctor on a regular basis. The doctor
will rely on not only test and exam results but also your honest
input when deciding any changes to your diabetes management
plan.
